New Patients
We welcome new patients to our practice. Please complete the patient questionnaire upon your first visit and hand it to our reception staff. Please ensure all sections are completed.
Appointments
Scheduling: Our practice operates on an appointment schedule. Consultations typically range from 15 to 30 minutes. Please specify if you require a short or long appointment when booking.
If You Are Unwell: If you have respiratory symptoms, please notify us so we can arrange a telephone appointment. If your doctor requires a face-to-face consultation, we ask that you please wear a face mask.
Cancellation Policy: We ask that you provide at least 24 hours’ notice for standard appointments (15 min) and 48 hours for long appointments (30+ min).
Late cancellations or non-attendance may incur a fee of $60 (or the full fee for long appointments).
Continuity of Care
To ensure the highest standard of care, an appointment is required for the following:

A follow-up appointment after a specialist visit may be required.
Health Management

Chronic Disease Programs: We are committed to preventive health. Patients with chronic conditions (e.g., asthma, diabetes, high blood pressure) are encouraged to participate in our care plans. If you are on a care plan, you will receive reminders for regular health checks. Please notify reception which type of reminder you received when booking.

My Medicare: We request that all patients register for My Medicare. This allows our GPs to bill Medicare for long telephone consultations and care plans. Please ask reception for a registration form.

eHealth Records: If you have a ‘My Health Record,’ please inform your GP so we can activate it in our system. For more information, visit

Privacy

Transfer of Records: If you require a transfer of medical records, please speak with our reception staff.
